When a patient comes in for a new breast augmentation consultation, it is first and most important to understand their motivations. Once we have achieved that, we like to get a better sense of their style and what they find to be attractive. My part of the equation begins with understanding the physical nature of their body and the balance that we can achieve. Our philosophy on breast augmentation includes the concept of biodimensional analysis. That is, breast implant size and shape should be chosen to enhance your breast and not simply replace your breast. The first component of any discussion on breast implant size begins with a comprehensive review of your health and breast history. After completing this health review, I and my staff will chronicle your breast history in terms of cup size, including the changes in breast size if you have accompanied pregnancy, weight gain, or weight loss. I think it's really important to remember at this time that cup sizes are not standardized and unfortunately cup size changes from brand to brand. Thus, we will try to discuss breast sizing in terms of more finite measures, like volume, width, and projection. The next step is recording a series of physical measurements which will characterize the dimensions of your chest, tissue amount, and position of your breast and your tissue laxity. We will discuss with you the significance of these measurements and explain how they will affect the choices that we make. Critical to breast implant selection is that there is adequate space and tissue coverage to support the size and weight of the implant. In our practice we place 99% of our implants under the muscle and while doing so, a simple rule must be observed. The implant width cannot exceed the width of the breast. Secondly, the implant's projection, how much it stands out from your chest cannot exceed the breast tissue's stretch. Thus, it is features like this of your own anatomy and features of the implants themselves that come into play in creating the optimal shape and the most pleasing aesthetics for you.
